Official Rules: Health Connector Survey Gift Card Drawing

The Commonwealth Health Insurance Connector Authority (“Health Connector”) is conducting a survey to find out more about how we can help more people enroll in coverage after they learn that they qualify through the Health Connector. Of the people who complete the survey, five (5) winners will be chosen to receive a $100 Visa Gift Card. Each winner must be a legal resident of Massachusetts, 18 years or older, and not currently enrolled through the Health Connector.

Rules

  • You must be 18 years of age or older at the date of completing the survey.

  • You may submit only one survey. Submitting more than one survey will disqualify you.
  • You must submit your entry through the survey link emailed to you from the Health Connector.
  • In order to be eligible to win one of the five (5) Visa Gift Card prizes, you must not be a current Health Connector member, and not currently be employed or previously have been employed by the Health Connector – whether as employee or contractor – or a member of the immediate family (parent, spouse, child, grandchild, or sibling) or a member of the same household as a Health Connector employee or contractor.
  • Your survey responses become the property of the Health Connector.
  • The five (5) Visa Gift Card winners will be selected at random by the Health Connector. The selection of the winners will be final.

Prize

The five (5) Health Connector Gift Card Winners will receive a $100 Visa gift card. Approximate retail value of the prize is $100.

All costs, taxes, fees, and expenses associated with any element of the Prize not specifically addressed above are the sole responsibility of each Gift Card Winner. All taxes are the sole responsibility of each winner. Gift card restrictions may apply. Prizes cannot be transferred, substituted, or redeemed for cash.

How Winners will be Determined

Five (5) potential Health Connector Survey Gift Card Winners (the “Gift Card Winners”) will be selected in a random drawing from among all eligible entries on or about June 12, 2026, and will be contacted by email or phone on or about the same day. A prospective Gift Card Winner may be required to sign an affidavit of eligibility and release of liability, and return same, properly executed, within five (5) days of issuance of prize notification. If the Health Connector is unable to contact a prospective Gift Card Winner within three (3) days from the first notification attempt, if a prospective Gift Card Winner fails to complete and return all requested forms by the specified date, or if a prospective Gift Card Winner does not fulfill the requirements, his/her/their prize will be forfeited and an alternate prospective winner shall be selected.

The odds of winning depend upon the number of eligible entries received.

Timeline

  • Survey entries must be received by Sunday, May 17, 2026 at 11:59 PM ET.

  • The winners will be notified on or about June 12, 2026.
