What to Expect: Tax forms
Forms related to proof of health insurance coverage when filing taxes If you are enrolled in Health Connector coverage—even if it only part of the year—you will get important forms you will need to file your state and federal income taxes. If you had individual or family coverage that was not through a Catastrophic type plan, the Health Connector will send you a Form 1095-A to complete your federal income tax return. You will receive Form 1099-HC from your health insurer and you will need this form to complete your Massachusetts state income tax return. When you get these forms in the mail in January, put these in a safe place immediately. These forms are important and needed to prove your health insurance when filing taxes. You will need to bring these forms with you when you go to your tax preparer or have them ready when you prepare your own returns.
