Upload Documents (Proof)

When you apply for coverage, renew, or update your information, you may be asked to send proof of some information that could not be electronically verified. Your proof can be sent to the Health Connector and MassHealth By uploading online through your account. By Mail, By Fax, and Bringing documents in-person to a MassHealth Enrollment Center. If you need in-person assistance, you can get help from an Enrollment Assister near you. Find locations → Important: If you are sending proof, don't send the same document in more than one way. For example, don't fax and upload the same document for proof. How to upload your documents Step 1 Sign in to your online account. (If you don't have an online account or or locked out, call Customer Service for assistance.) Step 2 In the main menu of your account, go to "Documents." This will take you to the Manage Documents page. Step 3 On the Manage Documents page, click the "Upload Documents" button, which will take you to the upload interface. What are Catastrophic plans?

Catastrophic health plans meet all of the requirements applicable to other Health Connector plans, but they don’t cover any benefits other than 3 primary care visits per year before the plan’s deductible is met. The premium amount you pay each month for health care is generally lower than for other health plans, but the out-of-pocket costs for deductibles, co-payments, and co-insurance are generally higher. To qualify for a Catastrophic plan, you must be under 30 years old OR get a “hardship exemption” because the Marketplace determined that you’re unable to afford health coverage. Catastrophic health plans: For people under 30 or with certain exemptions Catastrophic health insurance plans have low monthly premiums and very high deductibles. They may be an affordable way to protect yourself from worst-case scenarios, like getting seriously sick or injured. But you pay most routine medical expenses yourself. Who can buy a Catastrophic plan Only the following people are eligible: People under 30 People of any age with a hardship exemption or affordability exemption (based on Marketplace or job-based insurance being unaffordable, including those ineligible for APTC or CSRs) As of November 1, 2025, individuals who are ineligible for advance premium tax credits (APTC) or cost-sharing reductions (CSR) due to their projected annual income will be eligible for a hardship waiver. What happens if you or some of your household members qualify for MassHealth coverage?

The Health Connector website is the allows Massachusetts residents to use just one website to apply for health coverage in Massachusetts that includes public programs like MassHealth, the Children’s Medical Security Plan, and the Health Safety Net. If you or some of your household members have been approved for coverage through MassHealth, those people will get a letter from MassHealth in 3–5 business days with more information about their coverage and health plan options available. People who qualify for MassHealth coverage after completing the online application cannot shop for Health Connector or ConnectorCare plans.
