Form 1095-A Copies and Corrections

Form 1095-A shows the coverage through the Health Connector in 2023, and any Advance Premium Tax Credit you received for that year. This form helps you fill in IRS Form 8962 when you file your taxes. Request Form 1095-A corrections

If you think information on your Form 1095-A from the Health Connector is not correct, please call Customer Service as soon as possible at 1-877-MA ENROLL (1-877-623-6765) or TTY: 1-877-623-7773, during normal business hours.

Form 1095-A corrections cannot be requested online. Please call customer service.

Important Information About Form 1095-A Correction Requests

Form 1095-A shows the coverage through the Health Connector in 2023, and any Advance Premium Tax Credit you received for that year. This form helps you fill in Form 8962 when you file your taxes.

If your form doesn’t accurately show the coverage information we had on file about you—for example, if you had coverage through the Health Connector from January 2023 through October 2023, and your Form 1095-A only shows coverage from January 2023 through August 2023—then please contact us for a corrected form.

Before you request a correction, you may not need a corrected Form 1095-A if  any of these changes apply to you

Before you call to request a correction, please note that there is some information that may not need to be updated on your Form 1095-A in order to file your taxes.

  • Misspelled names. You may not need to correct a misspelled name on your Form 1095-A in order to file your taxes.
  • You moved to a different address in 2023 and you didn’t tell us, you may need to get a different Second Lowest Cost Silver Plan (SLCSP) premium amount from our calculator for those months at your new address and recalculate your SLCSP when you file your taxes. You may not need a corrected Form 1095-A from the Health Connector in order to file your taxes.
  • Your income went up or down in 2023 and you did not report it to the Health Connector. Just enter the correct information when you complete your tax return. You may not need a corrected Form 1095-A from the Health Connector.
  • You got divorced in 2023 and you didn’t tell us, you and your spouse should divide your premiums and APTCs between yourselves when you do your taxes. You may not need a corrected Form 1095-A from the Health Connector.
  • You had other changes in your household in 2023, like adding or losing a household member through marriage, birth, adoption, or other life change and you didn’t tell us, you may not need a corrected Form 1095-A from the Health Connector in order to file your taxes. You can get an updated Second Lowest Cost Silver Plan (SLCSP) premium amount from our calculator and recalculate your SLCSP when you file your taxes.

Download a copy of your Form 1095-A

To download a copy of your tax form, you will need to sign into your account.

  1. First, go to the account homepage and sign in
  2. Click the “Make a Payment” button OR scroll down to the “Health Connector Billing and Notices” section and click the “Go” button to open the member portal.
  3. Click on “Tax Forms” in the member portal menu to find your Form 1095-A available for download.

Tip: Make sure your browser’s pop-up blocker is disabled or allows pop-ups from MAhealthconnector.org.
